Our clinicians treat patients on both a temporary and permanent basis. Our Physician(s) will be your Primary Care Physician. We provide individualized in-home care for patients who suffer from illness, Injury or mobility issues. Our typical patients are Seniors or adults with disabilities who lack the ability to travel to a physician’s office or to access regular medical care outside their homes.

A team of physicians and mid-level clinicians (Nurse Practitioners and Physician Assistants) provide onsite care at regular intervals, based on clinical need. Between visits, our office staff is available to answer any questions or concerns, schedule urgent visits, refill medications, and coordinate other needed clinical services.

Patients and families who wish to utilize our online portal, are given secure User IDs and passwords, enabling them to access the Portal to schedule appointments, ask questions, and view personal and private documents including lab and diagnostic test results.

Who Qualifies for Housecalls

Medicare covers house calls for individuals who have difficulty leaving their home to access medical care without a “significant or taxing effort” and typically requires the assistance of another person or specialized transportation to leave home.

Insurance we accept:

Medicare Part B, Medicare Railroad, Aetna, Amerigroup Amerivantage, Blue Cross Blue Shield, Care n Care, Humana, United Healthcare and Private Pay.
